If you wish to receive treatment at our Cho Treatment Center, you are first required to attend a clinic for a written consent from a doctor. Treatment cannot be performed unless you have this consent form.

For First-Time Visitors
1.If you are attending our clinic for the first time, please present your health insurance card and medical certificate at the reception desk.2.You will then receive a medical application form and health questionnaire. Please fill this out and return it to the reception desk.
3.We will call you in order so please enter the examination room as directed by the nurse or medical staff.
4.After the examination, please wait in the waiting room. If you have a prescription, this will be handed to you at reception once payment is completed. Please take it to a nearby pharmacy to collect your medication.
